Discuss patient’s understanding, expectations and goals

Empower family members with understanding of symptoms and their management

Decision-making

Agree shared plan of action with patient based upon patient preferences, treatment guidelines, and multi-disciplinary team experience

Delivery

Deliver and co-ordinate a multi-disciplinary approach based upon good understanding of other specialities and roles
